Default double seal inspection chamber / damp membrane

I require a double seal inspection chamber in my extension floor slab,
where soil pipes from upstairs and downstairs loos will combine. Since
the bottom of this may be below the DPM,
how is the DPM and chamber "joined" in a damp-proof fashion ?
Also, what about insulation under/around the chamber ?
Cheers,
Simon.
