up in the ER after a serious accident or medical emergency
and you only find out about it after they have died etc.

Unlikely, all the emergency people, doctors etc. are well used to
dealing with these systems. All they need do is listen to the
announcement the system makes and follow the simple instruction.

The nuisance callers tend to use computers dialling numbers, which do
not follow the instruction - hence they are blocked.
