Default ID SMD chip - Regulator, DC converter?

Or more likely one of these, but I can't get any of my package markings to
match up with any of theirs.

Which may now be a moot point, as the problem seems to be 12v coming from
the circuitry that is meant to be supplied the 2.5v from this regulator.

Something is very dead.
