Problems even when the paper ballots were still counted by hand. And
fraud, pretty much any Chicago election in the 50s and 60s.
I have since managed to lose the bookmark, but there was a California
Supreme Court decision that allowed the recall election to go ahead that
included an absolutely fascinating discussion of the error factors
involved in the various types of vote counts. The most interesting to me
was that all they looked at (hole punch, the ones that look like SAT
cards, even hand counted) had roughly the same error rates.
