Default STK465

I've repaired a few amps for family and friends where the hybrid chip eg
STK465 has failed in one channel.
What causes this to happen? Are the chips inherently unstable? Is there
something the user can do or refrain from doing to prevent the fault
occurring?
(I'm not a tech but a fiddler/hobbyist. Don't do repairs for money, just
for "fun" and as a break from work routine. I'm a High School Librarian and
Computer Network Admin.)
