I haven't seen concentric cable for forty years.
It WAS a dead loss.
My house is supplied with it, including the cable they recently replaced.
So's mine - 2 core (live centre and neutral outer) on a TN-C-S system.
There is a 3 conductor version for TN-S systems too.
My earth is connected to the neutral at the meter. They are one and the same. No doubt the substation has an earth spike. I don't think I do, apart from the odd little wire to a pipe.
