There's no simple answer!

I suggest that you beg, borrow or steal a copy of Laurie Williamson's
excellent book on "Planning, Managing and Completing your Extension"
- then you'll have a much better understanding of the whole process.

P.S. It's usually better to build extensions bottom up! g
